This is a carved boar's tusk with a curly koa wood cap and has a kakau design.
The triangle symbols represent the niho mano(shark tooth). The nihomano signify strength and protection on one's life's path. The Diamond symbol is Na pae'aina/islands, that link one to Hawaii, with the lines below reprsenting one's ancestral iwi/bones and those ties. The graduating size of the symbols evoke generational ties to these islands.
